How United Concordia PPO plans usually work
- Preventive care such as exams, cleanings and X-rays is often covered at the highest level.
- Basic care such as fillings is usually covered at a percentage after your deductible.
- Major care such as crowns and bridges is usually covered at a lower percentage, up to your annual maximum.
Every employer plan is different. We check your exact plan, deductible and remaining maximum before treatment.
Use your benefits before they reset
Most plans reset on January 1. Unused cleanings and annual maximums do not roll over, so scheduling before the end of the year helps you get the full value of your coverage. Learn more.
